Welcome to Adoption Home Studies of Colorado secure online application. Completion and submission of this application is required for all clients who seek to engage in Home Study or Adoption Related Services, including returning clients. The application is organized into pages which will be saved as you go; please make note of the email address and password you created to login - if you are unable to complete the application in one sitting, or if you need to correct or add information, you will be able to save and log in again.

Before completion and submission of this application, you will be provided with a Disclosure Statement and Perjury Statement which you will need to read and acknowledge receipt of. These documents will again be provided to you for signature upon initiation of services. If you prefer to receive the Disclosure Statement prior to beginning this application, and have not yet engaged with a member of our staff, please contact us for more information.

After completing the application, you are encouraged to print a copy for your records and future use. Your application will not be submitted until you click the "Pay Fees" button to be directed to our secure online payment page to submit the $550 non-refundable application fee. Upon submission, you will receive a confirmation email that your application has been received and is under review. All information provided is securely stored and kept confidential.

By completing and submitting this application and commiserate fee, I / We, the foregoing applicant(s), acknowledge that any applicant who knowingly or willfully makes a false statement of any material fact or thing in this application is guilty of perjury in the second (2nd) degree as defined in Section 18-8-503, C.R.S., and, upon conviction thereof, shall be punished accordingly.
